Shipping Forms

To avoid delays with customs, please include THREE copies of the commercial invoice on the outside of the shipping package. The commercial invoice should list the contents as "plastic PRS™-probes" and the country of manufacture is Canada and probes are returning to Canada (Code 066).

For all international shipments, applicable import customs fees and/or duties are to be paid by the researcher
